This is why United Church Homes provides additional support services to our residents living independently to help them live their best life. Experienced service coordinators act as a personal advisor to residents and their families, helping to connect them with existing services in their local community such as transportation, healthcare and more.
UCH Cares: Home Health and Home Care
UCH Cares offers independent living residents home-based support and personal care services, allowing them to live abundantly on their terms. Whether the need is temporary or long-term, our personal care coordinators provide flexible, tailored plans that are designed to change as residents’ needs change.
Our menu of services for home care includes errands and shopping, light housework, scheduling and appointment assistance, socialization opportunities, exercise and fitness assistance, meal prep, delivery and much more. Our UCH Cares services are tailored to meet the unique needs of each resident we’re honored to serve.
In addition to our customizable home care services, UCH Cares also offers medical home health services, including assistance with medication and personal care.
Ohio’s Hospice at United Church Homes
If you or someone you love are nearing the end of life’s journey, hospice care services can be an unexpected blessing in providing the comfort, care and respectful support needed to lessen pain, promote quality of life and ease the transition process for all involved.
Ohio’s Hospice at United Church Homes offers hospice services to residents facing life-limiting illnesses in our United Church Homes communities. We are proud to partner with Ohio’s Hospice to offer our residents access to a full spectrum of quality care services and supports. The Ohio Hospice team includes board-certified hospice and palliative care physicians, hospice-certified nurses, state tested nursing assistants, social workers, chaplains and volunteers.
