United Church Homes Receives Generous Grants to Support NaviGuide Subsidies for Older Adults in Columbus
Columbus, OH (Apr. 16, 2024) United Church Homes (UCH), a leading national senior living provider, is pleased to announce the receipt of two significant grants in 2023 to support its NaviGuide program, aimed at assisting older adults and their families in navigating senior services. The grants, totaling $19,000, were awarded by The Columbus Foundation and The Harry C. Moores Foundation.
The Columbus Foundation granted $14,000 to UCH, sourced from two funds: The Harold and Eleanor Johnson Fund and PHA Financial Advocates Fund of The Columbus Foundation. This contribution has been instrumental in providing NaviGuide subsidies to 48 older adults in Franklin and Delaware Counties for a duration of three months each.
Additionally, The Harry C. Moores Foundation generously provided a $5,000 grant, which was utilized to extend subsidies to 17 older adults in Franklin and contiguous counties for the same three-month period. This support has enabled older adults to access vital service coordination free of charge, thereby alleviating the stress associated with navigating senior services.
“We are immensely grateful for the generous support,” said Kim Yoder, NaviGuide Program Director. “These grants have enabled us to extend our NaviGuide program to more older adults in the Columbus area, ensuring they receive the assistance and support they need to navigate the complexities of aging.”
NaviGuide, United Church Homes’ service coordination service in Ohio, aims to provide affordable assistance to older adults and their families in recognizing and addressing social determinants of health. Through personalized care planning, comprehensive needs assessments, and service linkage, NaviGuide supported more than 250 older adults in achieving optimal health and independence in 2023.
“We are confident that the NaviGuide program will significantly improve the mental health and well-being of the older adults we serve,” added Yoder. “With these grants, we are able to extend our reach and make a meaningful impact on the lives of older adults in our community.”
