Our Levels of Care
Assisted living at SEM Haven is a great place for seniors to start the next chapter of their lives. Our assisted living residents are encouraged to maintain maximum independence, while our friendly, experienced staff support them with their daily needs.
Our focus is on the individual and their quality of life. We provide regular social events and meaningful activities to enjoy. Assisted living is designed for those who require supervision or assistance to maintain their safety and independence.
Memory care at SEM Haven is a comforting, empowering experience. We have three households dedicated to those living with Alzheimer’s disease or other forms of dementia. We also offer two distinct levels of senior memory care for our assisted living and skilled nursing residents.
Our memory care homes feature spacious, comfortable living rooms, kitchens, dining rooms and outdoor patios with gazebos. Continuity of care is a focus, with consistent staffing and round-the-clock support.
Fast Track Rehab at SEM Haven provides professional nursing and therapy care for those recovering from a surgery, illness, or hospitalization.
Our therapists and nurses are experts in the rehabilitation process and have extensive experience working with seniors on their rehab goals. We promote healing and strengthening by helping our guests achieve their goals and return home. Our rehabilitation care is designed to help patients regain mobility and function so they can live a more enriching life.
SEM Haven offers long-term skilled nursing care at its best. Our household model of care is a wonderful way of life for our residents who, together with their family members and our staff, feel empowered to make decisions about their care and life. Each household has specialized healthcare and medical staff who work together to provide individual care in a positive and caring environment for our residents.
All SEM Haven caregivers are educated and trained on how to provide the best care for those residents living with Parkinson’s and other neurological disorders. In addition, SEM Haven’s therapy department received individual certifications to treat Parkinson’s and other neurological disorders using the Lee Silverman Voice Treatment (LSVT) program.
The LSVT program is an inpatient or outpatient program, completed one hour per day, four days per week, for four weeks, including all three therapies (speech, physical, and occupational) to better manage PD symptoms.
