Ohio’s Hospice and United Church Homes have joined together in a nonprofit, Medicare-certified hospice program, serving residents facing life-limiting illnesses in many of our United Church Homes communities. Through this relationship, a team-oriented approach is utilized, coming together to provide excellent care and services to residents and families.

Comprehensive Levels of Hospice Care

Hospice care is designed to address the varied needs of those who stand to benefit from it, as well as those of their loved ones. That’s why Ohio’s Hospice care services offered through UCH offer four levels of care—to meet the unique needs of everyone we serve.

The four levels of hospice care provided through our partnership with Ohio’s Hospice are—

  • Routine Care: Patients receive hospice services in their home setting.
  • Crisis Care: Patients receive care on a short-term continuous basis to monitor and manage uncontrolled symptoms.
  • General Inpatient Care: Patients receive care on a short-term basis in an inpatient facility to help relieve uncontrolled symptoms.
  • Respite Care: Patients receive care in a facility on a short-term basis, for up to five days, to provide relief to a caregiver.
